Evolve Medical was established with the goal of making quality healthcare more accessible. We provide a weight management program and aesthetic services.
Within our weight loss program, patients are treated by an obesity medicine specialist who takes a comprehensive, scientific approach to treating those who suffer from overweight & obesity.
Our practice focuses on preventative medicine & takes an individualized approach to help each patient achieve their health & weight goals throughout multiple states. Our priority is building strong patient-provider relationships.

From Initial Consultation to Long-Term Success
Comprehensive Weight Management Program
Initial Visit
This visit includes comprehensive medical & weight evaluation, individualized nutrition plan with education, & prescription for medication if appropriate.
Follow-up - Weight Loss Phase
Each 20-30 minute session monitors your progress, adjusting your personalized plan as needed. Monthly visits evaluate your health and adherence to your diet and exercise regimen, ensuring you stay on track towards your weight goals.
Follow-up - Weight Maintenance
Minimum follow-up every 3 months – as required by the Florida Board of Medicine. After reaching your weight loss goal, a plan for maintaining your weight loss is developed to meet your specific needs. Follow-up interval varies and is determined between the provider & patient.
Currently accepting patients by appointment only.
A credit card is required to book your appointment, however, you will not be charged unless you are a no call/no show for your appointment ($100 fee).
If you need to cancel or reschedule please do so at least 24 hours in advance, otherwise a no show fee will be applied and must be paid before rescheduling.
Evolve Medical does not accept insurance for office visits. We accept cash or card. No checks please. You may use your insurance for prescriptions and/or labs completed through our office. Please bring your insurance card to your initial visit. You may also request a superbill to submit to your insurance to request reimbursement for the cost of your office visit.
